We rebuilt Lumen's patient platform on a compliant architecture and cut clinician task times by 40% — while migrating 60,000 active records with no downtime.
Where they were when we started.
Lumen's platform had grown organically over five years and was showing every year of it. Clinicians were spending an average of fourteen minutes on a task that should have taken four, and the company had lost two enterprise deals over compliance gaps they couldn't close on the existing architecture.
A rebuild had been proposed twice internally and shelved both times, because nobody could see a path that didn't involve taking the platform offline.

What we actually did.
Strangle, don't replace
We put the new application behind the same domain and migrated one workflow at a time, routing traffic per-route. At no point was there a big-bang cutover. The old system kept serving what hadn't moved yet, and each migrated workflow was validated with real clinicians before the next began.
Compliance as architecture
Rather than bolting audit logging onto each feature, we built it into the data access layer — every read and write of patient data is logged centrally, with no way for a feature to opt out. Role-based access moved from per-screen checks to a single policy layer. Encryption at rest and in transit, session handling and data retention were specified before feature work started.
Design against the clock
We shadowed clinicians for two days and timed every step. The redesign collapsed a seven-screen intake flow into two, with the most common values pre-filled from context. That single change accounted for most of the eventual time saving.
The rebuild everyone had been afraid of turned out to be safe once we stopped treating it as one event and started treating it as forty small ones.
What changed.
Median clinician task time fell from fourteen minutes to eight. The compliance work unblocked three enterprise contracts that had previously stalled, two of which had been dead for over a year. Migration completed with no unplanned downtime.
